A Department of Transportation (DOT) exam is a specialized medical examination mandated by the Federal Motor Carrier Safety Administration (FMCSA). Its primary purpose is to ensure that commercial motor vehicle (CMV) drivers are physically, mentally, and emotionally fit to safely operate large vehicles such as semi-trucks, buses, or vehicles carrying hazardous materials. Unlike a general wellness checkup, this exam is job-specific and focuses on health standards that protect both the driver and the general public.

You are generally required to pass this exam if you:
Operate a vehicle with a gross combination weight rating of over 10,001 pounds.
Transport more than 8 passengers (for compensation) or more than 15 passengers (not for compensation).
Transport hazardous materials that require your vehicle to display a warning placard.

The exam must be performed by a Certified Medical Examiner (CME) listed on the FMCSA National Registry. During the appointment, the examiner will conduct:
Medical History Review: A thorough discussion of past surgeries, chronic conditions, and current medications.
Vision & Hearing Tests: You must have at least 20/40 vision in each eye (with or without corrective lenses) and be able to hear a "forced whisper" from five feet away.
Vital Signs: Checks for blood pressure (ideally below 140/90) and pulse rate to assess cardiovascular health.
Urinalysis: A sample is collected to screen for underlying conditions like diabetes or kidney disease. Note: This is typically a medical screening, though some employers may require a separate drug test during the same visit.
Physical Systems Exam: An evaluation of your heart, lungs, neurological function, and musculoskeletal strength to ensure you can handle the physical demands of the job.

A valid Driver’s License or photo ID.
A complete list of medications, including dosages and prescribing doctors.
Any corrective devices like glasses, contact lenses, or hearing aids.
Specialist clearance letters for chronic conditions (e.g., a letter from a cardiologist if you have heart issues or CPAP logs for sleep apnea).
